If you are a patient, we will not be collecting your personal information, such as your name, address, email, or phone number. If you sign in to the external billing portal (https://www.patientnotebook.com/sagis/payment) to make payments, your personal information may be stored on the external billing portal. It will not be stored on our site.

We use the information we collect about you to deliver the services we offer, respond to you, and operate and improve our sites and applications. Our services include showing you the types of medical services that we provide, access to a secure external portal for making online payments and processing online order requests for supplies, laboratory material request forms, client forms and otherwise; sign-up sheets to voluntarily receive emails about our events and academic programs.
